We are seeking a highly skilled and compassionate Telehealth Nurse Practitioner to provide top-notch patient care and support our organization's mission to improve healthcare outcomes through digital health solutions. As a Telehealth Nurse Practitioner with Path Care Health, you will play a vital role in our virtual care team, working closely with patients, healthcare providers, and other team members to deliver exceptional care.

Responsibilities:

• Conduct virtual health assessments, diagnose and treat patients remotely, and develop personalized treatment plans

• Collaborate with healthcare providers and other team members to ensure seamless patient care

• Provide patient education and promote health literacy through virtual sessions

• Document patient encounters and maintain accurate, up-to-date records

• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

• Stay current with industry developments and best practices in telehealth and patient care

Requirements:

• Doctor of Nurse Practice (DNP) or Master of Science in Nursing (MSN) degree

• Current certification as a Nurse Practitioner (NP)

• Minimum 2 years of experience in primary care or related field

• Ability to work independently and as part of a virtual team

• Excellent communication and interpersonal skills

• Proficiency in electronic health records (EHRs) and telehealth platforms

• Strong analytical and problem-solving skills
