Homeless Patient Advisor

Providing specialist advice and practical support to patients that are homeless or in housing need and in hospital.

Offering advice and support to ward staff to facilitate a timely and safe discharge to homeless patients.

Visiting patients on the ward to understand their housing needs.

Ensuring the Trusts met their duty to refer under the Homelessness Reduction Act.

Liaising with community partners and referring to accommodation providers to support arranging a safe discharge.

• Previous relevant housing/ homelessness experience.
• Experience of working with those with complex needs
• Experience of working in the NHS

• Ability to prioritise and manage own workload
• Excellent verbal and non-verbal communication skills
• Up to date knowledge of housing provisions
• Housing or advice training
• Knowledge of local initiatives relating to homeless strategies
