Lander Medical Practice is seeking a motivated and experienced clinician to join our forward thinking, multidisciplinary team as a Long Term Condition Practice Nurse for 25.5hrs per week. This role will be based across our Health Park site in Truro city centre and our branch surgery in Threemilestone.
We care for more than 19,000 patients, and we are looking for a skilled nurse who is committed to delivering high quality care and eager to continue developing professionally within a supportive and progressive environment.
The successful candidate will have at least two years post registration experience in managing long term conditions
Confidence working autonomously as well as collaboratively within a multidisciplinary team
A proactive, patient centred approach and a commitment to continuous improvement

Main duties of the jobWe are seeking an experienced, compassionate clinician to deliver high quality, evidence based care for patients with long term conditions, including asthma, COPD, diabetes, and cardiovascular disease. The ideal candidate will bring strong clinical expertise and a patient centred approach to managing chronic conditions within a busy primary care setting.
The ability to undertake general nursing duties such as wound care, immunisations, cervical screening, and treatment room tasks would be required.
About usLander Medical Practice presently has team of dedicated GPs, nurses, physiotherapists, HCA's and many ARRS roles employees. Together with our neighbouring practice, we form Truro PCN. We are located within Truro Health Park and also have a satellite surgery based in Threemilestone.
Job responsibilitiesThe successful applicant will provide appropriate care and treatment for our practice population, following Practice policies and protocols for our patients with Long Term Conditions.

Person Specification Knowledge- Knowledge of managing patients with long term conditions including Diabetes, Asthma and COPD
- Two years post-registration experience in Long Term Condition management.
- High level of clinical skills including family planning/woman's health, immunisation and wound management
- Excellent communication skills, both written and verbal
- Experience of working within a GP practice
- Cervical cytology
