You will work alongside our Nurse Consultant, care coordinators, clinicians, Pharmacy Technician, OT, and community nursing teams to support frail, long-term or acutely housebound patients and nursing/residential homes within our Integrated Neighbourhood.
With knowledge in frailty, physical assessment and clinical reasoning, you will use these skills to assist in recognizing early symptoms
