The Millbrook Education Department is in search of an organized Educational Transition Support Person dedicated to collaborating with Truro Elementary School. As a critical link between Millbrook students, their families, and TES, you will provide the necessary support for a successful transition into public schooling. Your primary aim will be ensuring students feel confident and accepted in their new academic environment.

Key Responsibilities:

• Design individualized transition support plans

• Assist students in adapting to new classroom environments

• Promote confidence in navigating academic expectations

• Foster inclusive social interactions among students

• Strengthen family connections with school personnel
