Reports To: TLP Program Manager – Residential Programs Department:

Location: Tucker, GA

FSLA: Non-Exempt

Base pay range: $16.00/hr - $16.00/hr

Job Summary:

The Transition Support provides direct support to the Life Coach and Transition Support Team and maintains the community living skills and support for young adults as they develop a higher level of independence. The Transition Support role typically provides services to youth on a day to day basis that support permanency, safety and well‑being. The position also supports clients with life skills training, watchful oversight, support and guidance so they may attain a level of independence to live on their own.

Responsibilities:

  • Supervises/monitors youth’s progress, based on youth’s skills development and level of maturity as outlines in the client’s Supervision and Independence and Safety and Crisis management plans
  • Orient new clients upon entry in the program
  • Work in partnership with the Wellroot Life Coaches and other staff to develop and implement a plan for clients
  • Teach and support individualized life skills according to the ISSP, CLSA results, and based on youth’s need. Life skills including but not limited to grocery shopping, self‑care, daily living tasks, communications, budgeting, community living and socialization and community mobility
  • Document at least four (4) efforts weekly individualized independent living skills development activities for each youth on assigned Transition Support Team that are aligned with the ISSP goals
  • Offer job search training in areas such as resume writing and interviewing
  • Provide budgeting and daily living skills by supporting and monitoring weekly budgeting
  • Provide applicable phone numbers to TLP youth in order to ensure twenty‑four (24) hour access to appropriate personnel within the agency
  • Appropriately maintain keys to each TLP youth’s housing to ensure twenty‑four (24) hour access to all TLP residences
  • Ensuring and enforcing program expectations to ensure safety, growth and life skill development of the youth in the program
  • Assist clients in establishing and maintaining involvement in community recreation and productive leisure time activities
  • Encourage the spiritual growth of each client by connecting them with spiritual resources on staff or in the community
  • Teach within an evidence informed practice to encourage and supervise the reduction/elimination of maladaptive behaviors and follow individualized skills and service plans for clients
  • Ensure that supervision is designed to allow the agency observation of the youth is practicing healthy life skills and sound decision‑making
  • Participates in service meetings with other personnel as appropriate
  • Maintain required RBWO standards of supervision of residents at all times
  • Support and maintain an environment of emotional and physical safety for clients
  • Documents all reportable and non‑reportable incidents that occur with clients and notify the appropriate parties within timeframe outlines in program policy
  • Increase the client’s social and financial independence resulting in decreased oversight to ensure self‑sufficiency
  • Provide schedules, routines environments and engagement of youth in the program
  • Assist clients in obtaining medical services
  • Transport clients to necessary medical, dental, mental health, educational appointments and job interviews/career fairs
  • Transport youth to or from work or school when necessary
  • Complete required record‑keeping and documentation accurately and in a timely manner
  • Maintain confidentiality of client information
  • Maintain appropriate professional boundaries
  • Utilize a percentage of hours on assigned caseload as determined by supervisor
  • Maintain required certifications and training hours
  • Adhere to the organization’s policies and procedures, and demonstrate support of the mission, vision and values
  • Administer life skills assessments at prescribed intervals
