Summary

Clinical Pharmacist is responsible for providing clinical services in an inpatient setting. Responsibilities include collaboration with nursing and medical staff for medication chart reviews to optimize medication therapy, implementing up-to-date pharmacotherapy and treatment guidelines with providers, conducting Drug Utilization Reviews, and preparing Drug Review Criteria for medical staff review at the Pharmacy & Therapeutics Committee. The Clinical Pharmacist is also an active preceptor for PGY1 residents, pharmacy students, and pharmacy staff.

Functions
  • Majority of shifts scheduled will be clinical shifts.
  • Interprets and clarifies written orders for all types of medications.
  • Prepares compounds, labels, and packages medications and pharmaceuticals, in accordance with established professional protocols and standards.
  • Secures storage of narcotics and other controlled substances on a regular basis.
  • Ensures all charges for dispensed medication orders are captured in hospital’s billing system.
  • Provides medications and drug information relative to circumstances and patient conditions in emergency codes in the hospital.
  • Communicates drug‑related side effects, dosages, potential complications to health care professionals.
  • Advise patients and/or their representatives on use of various medications, including dosage, side effects and composition.
  • Remains informed about current developments concerning new drugs or test results regarding drug usage; researches drug information requests and responds promptly.
  • Promotes effective use of formulary system through drug product selection and therapeutic alternative suggestions for non‑formulary orders.
  • Monitors drug therapy as assigned by physicians; suggests alternative drug therapy when appropriate.
  • Consults with and advises medical staff personnel regarding utilization of drugs and pharmaceuticals.
  • Oversees work of Pharmacy Technicians/Pharmacy Interns participating in the preparation, documentation and dispensing of medications and pharmaceuticals.
  • Adheres to TMCH organizational and department‑specific safety, confidentiality, values policies and standards.
  • Provides education to pharmacy interns, students and PGY1 residents.
  • Participates in rounds and care coordination conferences as appropriate.
  • Participates in quality improvement committees as assigned.
  • Performs related duties as assigned.
Role Specific Functions
  • TPN Pharmacist – Provides comprehensive patient assessment, ordering and monitoring of parenteral nutrition therapy including fluid and electrolyte status, glycemic management and related drug therapies as specified in MEC‑approved parenteral nutrition procedure. TPN pharmacists are involved in the management of all adult, pediatric and neonatal patients requiring parenteral nutrition.
  • Pain Team Pharmacist – Provides comprehensive patient assessment, ordering and monitoring of oral and intravenous analgesics as specified in MEC‑approved pain consultation procedure. Pain team pharmacists are involved in the management of all adult and pediatric patients via consult service.
  • Glycemic Care Team Pharmacist – Provides comprehensive patient assessment, intervention and monitoring of oral and injectable hypoglycemic agents for patients that meet high‑risk criteria.
  • Antimicrobial Stewardship Pharmacist – Provides comprehensive oversight of house‑wide antibiotic use. Recommends changes to antimicrobial therapy including drug choice and dosage adjustments. Gathers and analyzes data and reports out to Infection Control, Pharmacy and Therapeutics Committee (P&T) and Antimicrobial Stewardship Committee.
  • Oncology Pharmacist – Provides comprehensive patient assessment, intervention and monitoring of oral and intravenous antineoplastic agents in accordance with guidelines. Oncology pharmacists are involved in patient education for infusion center patients.
Minimum Qualifications
  • Education: Doctor of Pharmacy degree from an AACP‑accredited College of Pharmacy or Bachelor’s Degree from an AACP‑accredited College of Pharmacy.
