At CBI, a psychiatric practitioner is an integral specialty for our whole‑person approach in our integrated system of physical, psychiatric, and addiction services. The scope of practice includes adults 18+ in inpatient and outpatient settings, addressing complex medical history and co‑occurring psychiatric disorders. Practitioners will provide patient‑centered, evidence‑based care and serve as patient advocates in all health‑related matters, including the appropriate use of specialists, health services, and community resources.
General scope within their skills, competencies, and experience, licensed practitioners shall:
- Complete Medical Practitioner Admission Orders and CON (for a Physician only). If the practitioner is not a physician, they will coordinate with the CPEC Medical Director to sign the CON within 24‑hours of admission.
- Complete a psychiatric evaluation (HPI) in its entirety and document it in the patient file for each individual admitted to the inpatient behavioral health facility within 24‑hours of admission; review and acknowledge the completed physical health H&P within 24‑hours.
- Review and modify treatment plans to meet psychiatric, medical detoxification, and physical health needs.
- Review cases with RN and Transition Coordinator daily during rounds as part of discharge planning.
- Perform rounds daily in inpatient between the hours of 0800 and 1600.
- Document daily rounds in progress notes.
- Monitor the treatment of physical health and/or coordinate with PCP and other prescribers while in treatment.
- Initiate or recommend consults and coordination of care.
- Coordinate with other CBI programs or external facilities during discharge, admission, or transfer, documenting in the medical record to ensure a smooth transition.
- Be available or initiate coordination of care promptly; direct staff to obtain collateral information; review provided medical documentation and document findings.
- Provide necessary documentation and treatment for patients under petition or court‑ordered treatment, including Special Treatment Plan, and coordinate with the outpatient team.
- Issue discharge orders and plan within 24‑hours of discharge.
- Document and sign full discharge orders and follow‑up recommendations within 24‑hours of discharge.
- Review discharge plan with the patient.
- Prescribe appropriate pharmacotherapy for psychiatric disorders, coordinating care for physical health, substance abuse treatment, and pain management; adhere to accepted standards of care.
- Maintain at least 80% e‑prescribing; goal 100%.
- Incorporate addiction medicine protocols prior to discharge.
- Incorporate suboxone taper evaluation prior to discharge.
- Incorporate ongoing mental health/psychiatric care evaluation prior to discharge.
- Incorporate ongoing physical health evaluation prior to discharge.
- Complete orientation as directed by CBI, including shadowing a practitioner before working with patients.
- Complete TB skin test prior to working with CBI patients and annually thereafter; submit results to Credentialing annually.
- Obtain and maintain suboxone waiver to treat the maximum number of eligible patients.
- Maintain necessary patient data logs when treating patients with buprenorphine.
- Provide CPR/First Aid evidence prior to working with CBI patients; renew every two years and submit to Credentialing.
- Participate in quality management initiatives; correct deficiencies within 72 hours unless otherwise approved.
- Maintain at least 85% compliance with documentation requirements.
- Do not cancel patient appointments without prior notification to CBI.
