40 guaranteed hours/week

Day shift

8:00 AM–4:30 PM

Patient navigation and follow-up responsibilities

Focus on post-procedure patient support and education

Active RN license

Comfortable conducting post-procedure follow-up calls

Ability to educate patients and address questions or concerns

Strong documentation and care coordination skills

Previous patient navigation, outpatient, procedural, imaging, or care coordination experience preferred
