Travel Physical Therapist – Home Health - Location:
Tucson, AZ - Contract:
13 Weeks (08/17/2026 – 11/14/2026) - Schedule:
36 Hours/Week - Shift: Days (8:00 AM – 4:30 PM)
- Home Health experience preferred
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop and implement individualized treatment plans.
- Improve patients' mobility, strength, balance, and functional independence.
- Educate patients and caregivers on exercises, safety, and home programs.
- Complete timely and accurate documentation while coordinating care with the interdisciplinary team.
- Active Arizona Physical Therapist License
- Current BLS Certification
- Recent Physical Therapy experience in a home health or acute care setting
- Excellent assessment, documentation, and patient education skills
- Valid driver's license and reliable transportation for patient home visits
- Monday–Friday day schedule with a great work-life balance
