Responsibilities
  • Implements and monitors patient care plans; records and communicates patient condition as appropriate.
  • Serves as a primary coordinator of all disciplines for well‑coordinated patient care.
  • Notes and carries out physician and nursing orders.
  • Assesses and coordinates patient discharge planning needs with members of the healthcare team.
  • Assessment:
    Collect and analyze physiological, psychological, sociocultural, spiritual, economic, and lifestyle data.
    • Initial assessment
    • Re‑assessment
    • Focused assessment based on patient condition, setting and population
  • Problem Identification:
    Make clinical judgments about patient responses to health conditions or needs.
  • Planning and Outcomes:
    Set measurable goals and document in the patient’s care plan.
    • Establishing daily goals with the patient and family
    • Formulating a plan of care
    • Transition or discharge planning
  • Implementation:
    Deliver nursing care per the care plan and document in the record.
    • Ensures patient safety
    • Medication administration
    • Provision of ordered treatments
    • Patient/family education
    • Documentation in healthcare record
  • Evaluation:
    Continuously assess patient status and care effectiveness; modify care plan as needed.
  • Care Coordination:
    Manage timing and effectiveness of patient care from admission to discharge.
    • Delegation
    • Supervision
    • Prioritization
Position Summary

Provides direct nursing care in accordance with established policies, procedures, and protocols of the healthcare organization.

Minimum Requirements
  • Graduate of an accredited School of Nursing
  • License and

    Certifications:
    • Basic Life Support (BLS) – American Heart Association
    • Valid current Arizona Nursing License or compact state license with ability to become Arizona licensed within 30 days
Preferred Requirements
  • Bachelor of Science in Nursing
  • Certifications:
    • Advanced Cardiac Life Support (ACLS) – American Heart Association
    • Pediatric Advanced Life Support (PALS)
    • Neonatal Resuscitation Program (NRP)
    • Other certifications as required by individual department
