Job DescriptionMonogram Health is seeking skilled Nurse Practitioners and Physician Assistants to join a collaborative Care Team that includes registered nurses and social workers. The role focuses on patients with complex chronic conditions, primarily chronic kidney disease (CKD) and end‑stage renal disease (ESRD/ESKD). The organization delivers evidence‑based in‑home care and benefit management for highly complex patients.

Monogram Health is a leading multispecialty provider of in‑home, evidence‑based care for patients with multiple chronic conditions such as diabetes, hypertension, CKD, heart failure, depression, COPD, and other metabolic disorders. The team includes specialists in nephrology, cardiology, endocrinology, pulmonology, behavioral health, and palliative care. The practice operates 24/7 to support patients in their homes.
Roles and Responsibilities- Conduct comprehensive annual wellness exams in patients’ homes and via virtual platforms
- Counsel and educate patients and families about benefits and programs to promote healthier lives
- Document chief complaints, diagnoses, medical history, review of systems, examinations, medications, allergies, assessments, and plans
- Coordinate care with primary providers, specialists, and ancillary services
- Complete all documentation and paperwork promptly
- Maintain quality care standards as defined by the practice
- Deliver evidence‑based, timely care that reduces avoidable hospitalizations and maximizes patient satisfaction
- Prescribe medications, order tests, and collaborate with Monogram physicians
- Monitor patient outcomes daily and report improvements
- Educate patients on preventive monitoring and facilitate scheduling of diagnostic tests
- Assist patients with enrollment to educational resources
- Participate in integrated care team meetings
- Assess patient health status and develop treatment plans
- Implement plans consistent with the care plan
- Follow up and evaluate patient status
- Perform other duties as assigned
- Active, unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license
- Board certification in appropriate specialty (NP: ANCC/AANP; PA: NCCPA)
- Current, unrestricted DEA certificate
- Ability to work autonomously without direct supervision
- Valid driver’s license, car insurance, and reliable transportation
- Proficiency with required medical instruments and equipment
- Knowledge of computer‑based data management and electronic medical record systems
- Effective verbal and written communication with patients, families, and medical partners
- Sound understanding of federal and state regulations including HIPAA and OSHA
- Minimum two years of direct patient care experience
- Experience in managed care/IPA/health plan settings
- Experience conducting comprehensive wellness visits virtually or in the home
