As a Hospice RN Case Manager, you will coordinate and oversee care for an assigned group of patients while providing skilled nursing, education, symptom management, and emotional support to patients and their families.
Responsibilities Include:Provide compassionate hospice nursing care in patients' homes and other care settings throughout Greater Tucson
Complete comprehensive nursing assessments
Develop, implement, and update individualized plans of care
Manage pain and symptoms with a focus on comfort and quality of life
Monitor changes in patient condition and respond appropriately
Coordinate medications, treatments, supplies, and other patient needs
Educate patients and families regarding medications, symptom management, disease progression, and end-of-life care
Communicate and coordinate care with physicians and other healthcare providers
Collaborate with CNAs, social workers, chaplains, and other interdisciplinary team members
Participate in interdisciplinary group (IDG) meetings
Provide emotional support and guidance to patients and their loved ones
Maintain timely, accurate, and compliant clinical documentation
Help families understand what to expect throughout the hospice journey
Current Arizona RN license or applicable compact license
Compassionate and patient-centered approach to care
