Requirements:

• Current Nursing License in the practicing state
• Valid Driver's License
• Current CPR Certification
• Two years of experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting
• Proof of current Hepatitis consent/declination and TB vaccination record
• Ability to use electronic medical charting and time-keeping software
• Reliable transportation and current auto liability insurance coverage
• Ability to read, write and speak fluent English

Responsibilities:

• Administer skilled nursing care to patients in their residence as prescribed by the physician.
• Assist in providing nursing services and identifying the patient's needs by documenting observations and evaluations.
• Participate in planning, coordinating, and implementing care alongside the interdisciplinary team.
• Provide effective communication to patients/families, co-workers, and other health care professionals.
• Assist patients/families in learning appropriate self-care techniques.
• Maintain compliance with infection control measures.
• Assist with the admission, discharge, and transfer of patients.
• Perform other related duties as assigned.
