• Interview, hire, and regularly direct the work of five or more full-time equivalent direct support staff. Oversee enrollment in and completion of all training and in‑services required by DDS and the person’s team.
• Monitor the records and programs of the person receiving supports. Complete a monthly program review by the tenth of each month that documents the person’s progress and status.
• Participate as an active member of the person’s team, acting as a liaison to all professionals and interested parties.
• Complete the required agency and DDS number of on‑site monitoring visits, and complete required documentation of such visits.
• Attend and participate in agency and DDS meetings and training classes.
• Provide continuous on‑call emergency services to persons receiving supports and staff.
• Be knowledgeable of and adhere to DDS, contract and agency policy and procedures.
• Act as a Health Care Coordinator (and/or back‑up). Monitor routine and special medical needs and fulfill all HCC duties required by DDS policy.
• Assure ongoing eligibility for all benefits and report monthly wages to the appropriate agency.
• Monitor payment of all bills and expenditures.
• Keep continual contact with DHS‑DDS case managers, parents/guardians and other family members of the person receiving services.
• Provide direct support services on an as‑needed basis to the individual by completing all duties listed in the DSP Job Description. These direct supports may include providing transportation to people receiving services to their place of employment, to therapies/medical appointments or to leisure or activities using public transportation, agency‑leased vehicles or the employee’s own vehicle.
• Assist individuals to secure and attend volunteer as well as other community vocation activities in their community in order to prepare for competitive employment.
