Hours:

40 hrs/week | Shift: Mon-Fri 8a-4:30p | No Call

Pay: $40.00/hr - $48.00/hr | Travel: $1,950/week

• Provide skilled nursing care to patients in their homes under the supervision of an RN and in accordance with the care plan.
• Administer medications, treatments, and wound care while monitoring patient responses and reporting changes in condition.
• Educate patients and families on disease management, medication administration, and self-care techniques.
• Document all care, observations, and interventions accurately and timely per agency and regulatory requirements.
• Collaborate with physicians, RNs, and other healthcare professionals to ensure safe, effective, and coordinated patient care.

• 2 years experience as an LPN in a clinical setting, preferably in a home health or hospice setting
• Active LPN License
• Valid Driver's License
• BLS (AHA)
