Hospice Registered Nurse Case Manager
Tulsa, OK
Must Haves- Active RN license in the state of practice
- Minimum 1 year of hospice or home health experience
- Ability to manage a fast-paced, independent schedule
We are seeking experienced and compassionate Registered Nurse Case Managers with hospice experience to join our dedicated hospice team. This role is central to providing high-quality, patient-focused care while coordinating services between patients, families, and the interdisciplinary team.
Responsibilities- Provide skilled nursing visits to hospice patients in homes and facilities
- Coordinate care among patients, families, and the interdisciplinary team
- Assist with on-call duties as needed
- Conduct field and telehealth visits in compliance with regulations and patient needs
- Educate patients and caregivers on care plans, medications, DME, and supplies
- Complete patient admissions and all required Plan of Care and IDT documentation
- Degree in Nursing
- Current RN license in the practicing state
- Minimum 1 year of hospice or home health experience
- Strong interpersonal and communication skills
- Experience with Hospice and / or Home Health EMR systems is a plus
- Medical insurance through Blue Cross & Blue Shield
