• Active, unrestricted LPN/LVN license in the state of practice (compact licenses must be transferred within 90 days)
• Current CPR certification with hands‑on component (post‑hire support available)
• Recent TB skin test (within last 12 months)
• Minimum six months of hands‑on nursing experience preferred
• Reliable transportation
