Nurse Practitioner

Join our team as a Hospice and Palliative Care Nurse Practitioner at Agape Care Group.

• Provide medical management for seriously ill patients, addressing symptoms and aligning treatments with goals and values.
• Collaborate with interdisciplinary clinicians to deliver the best care at home or in facility settings.
• Lead palliative and hospice care services, including patient‑family education and support.
• Participate in community outreach and serve as a regional medical community member.

• A heart for patient care and a passion for the best possible outcomes.
• Graduate of an accredited Certificate, Diploma, Associate or Baccalaureate School of Nursing.
• At least 2 years’ experience in chronic disease management/long‑term care; hospice experience preferred.
• Reliable transportation and ability to sit, stand, bend, move intermittently and lift 80–100 lbs.
• Clinically competent and capable of caring for patients at end‑of‑life.
