Grace Hospice, a part of the Optum family, seeks a dedicated Registered Nurse Case Manager to deliver in-home hospice care in Tulsa, OK. You will assess patients, develop plans of care, and coordinate with the IDG to promote comfort and quality of life.
The role requires an Oklahoma RN license, a valid driver's license, and access to reliable transportation. Standard hours are Monday–Friday with on-call for evenings and weekends; travel within a 60-mile radius is expected.
