Spiritual Caregiver​/Chaplin & Bereavement Coordinator - Hospice

Summary

We are hiring for a Spiritual Caregiver/Chaplain.

This is a Part Time position doing Home Visits for Hospice Care. Bereavement Coordinator duties are also required.

Typically 20 hours/week. Flexible schedule options:
Mon-Fri half days or 2-3 full days/week.

Candidates must provide documentation of current licensing/ordination in accordance with the applicant’s spiritual/faith tradition. Faith groups must be recognized by the Armed Forces Chaplains Board.

The Spiritual Caregiver is responsible for providing spiritual care to Hospice patients and their families.

• Demonstrates understanding of the difference between being a rabbi/imam/pastor/priest/etc. and being a Spiritual Caregiver.
• Demonstrates proficiency in providing spiritual care to persons of all races, ages, genders, sexual orientation, social class and mental/emotional status
• Provide spiritual care to persons of beliefs that are different than the Spiritual Caregiver's beliefs by championing cultural and spiritual humility
• Demonstrates ability for patient/caregiver to lead the spiritual conversation, for the Spiritual Caregiver to walk alongside him/her in his/her spiritual journey and refraining from sharing one's own beliefs unless asked to do so by the patient/caregivers.
• Demonstrates knowledge of a wide range of spiritual resources to support each person's belief system, provide resources as appropriate, and network with other religious leaders in the community in order to refer patients/caregivers to these leaders when necessary.

The Bereavement Coordinator is a qualified professional who supervises the provision of bereavement services reflective of patient and family needs.

• Demonstrates proficiency in providing bereavement services to persons of all races, ages, genders, sexual orientation, spiritual beliefs, and mental/emotional status as evidenced by observation and EGSS scores.
• Establishes a Plan of Care that addresses bereavement needs with clear delineation of services to be provided and frequency of service delivery up to thirteen (13) months following the death of the patient.
• Demonstrates active listening and impactful suggestions to bereaved during phone and home visits within LHC-required time guidelines as evidenced by observation, Visit Notes and EGSS scores.
• Leads and coordinates at least 12 effective sessions of Grief Support Group per calendar year as evidenced by observation and EGSS scores.
• Maintains Bereavement Binder with information and attendance sheets from Memorial Services, Grief Support Groups, Holiday events, and other Hospice and Community groups and presentations.
• Demonstrates individualized and compassionate care for each bereaved as evidenced by documentation in Visit Notes, Bereavement Risk Assessment, Care Plan, and EGSS scores.

Education

• Preferred:
  Four units of Clinical Pastoral Education or Master’s degree in theology, divinity, pastoral care, philosophy, religion or related discipline.
• Required:
  
  Two units of Clinical Pastoral Education or Bachelor’s degree in theology, divinity, pastoral care, philosophy, religion or related discipline.

Experience

• Preferred:
  Two years Hospice experience.
• Required:
  
  A minimum of two years’ experience in a health care setting and/or related pastoral experience.

License Requirements

• Provide documentation of current endorsement or acceptable language (licensing, ordination, etc.) in accordance with the applicant’s spiritual/faith tradition. Faith groups must be recognized by the Armed Forces Chaplains Board
• Provide documentation that demonstrates that the candidate is in good standing with their spiritual/faith group with no ethical charges current or pending.

Skill Requirements

• Ability to minister to patients and families in crisis
• Exceptional verbal communication skills
• Ability to plan, organize, and exercise independent judgement
• Demonstrated ability to be open, sensitive, flexible and ecumenical.
• Current CPR, driver's license, valid vehicle insurance card and access to a dependable vehicle, or public transportation.
