Registered Nurse - RN

Purpose and Scope

The registered professional nurse Home Therapies RN CAP 1 is an entry-level designation into the Clinical Advancement Program (CAP). The position is accountable and responsible for the provision and coordination of clinically competent care, including assessment, planning, intervention, and evaluation for an assigned group of patients. The RN manages patients’ response to home dialysis training and therapy, follows prescribed protocols, and communicates patient-related issues to the physician as needed.

As part of the End Stage Kidney Disease (ESKD) team, the RN participates in decision-making, teaching, leadership, and quality improvement activities that enhance patient care outcomes and facility operations.

Principal Duties and Responsibilities
  • Perform all essential functions under the direction of the Supervisor, with guidance from the Educator, Preceptor, or collaboration with another RN.
  • Conduct ongoing systematic collection and analysis of dialysis data for assigned patients, document in the patient medical record, make adjustments to the treatment plan as indicated, and notify the Supervisor or physician as necessary.
  • Assess, collaborate, and document patient/family basic learning needs to provide initial and ongoing education.
  • Direct and provide, in collaboration with the patient, home care partner, direct and ancillary patient care staff, all aspects of the safe and effective delivery of dialysis therapy.
  • Administer medications per protocol, documenting justification and effectiveness.
  • Initiate or assist with emergency response measures.
  • Act as a resource for the health care team, participate in staff training and orientation of new staff as assigned.
  • Ensure correct laboratory collection, processing, and shipping procedures; reschedule missed or insufficient collections.
  • Identify expected outcomes, document and update the nursing assessment and plan of care through collaboration with the Interdisciplinary Team.
  • Ensure patient awareness related to transplant and treatment modality options.
  • Assist in identifying, evaluating, selecting, and educating Home Dialysis candidates and Home Partners.
  • Assess barriers of the Home Dialysis candidate’s home environment and partner/family readiness to perform dialysis treatments at home.
  • Train Home Dialysis patients and/or Home Partners on operation and maintenance of all Home Dialysis equipment and supplies through a formal training program.
  • Complete CAP requirements to advance.
  • Perform all other duties as assigned by Supervisor.
Physical Demands and Working Conditions
  • Direct patient care involving heavy lifting, moving equipment and patients, and assistance with ambulation.
  • Frequent, prolonged periods of standing with ability to bend over.
  • Move machinery and equipment up to 200 lbs.; lift chemical and water solutions up to 30 lbs. as high as 5 feet.
  • Work in a health care facility with air temperature control and moderate noise levels.
  • Exposure to infectious and contagious diseases/materials.
  • Rotate coverage with other licensed home therapies staff to ensure reliable coverage.
  • Participate in on-call rotation, night, weekend, holiday or as defined by program needs.
  • Travel to training sites, other facilities or patient homes as required.
  • Provide essential functions at other locations, including patient homes, with the same physical demands.
  • Day-to-day includes desk work, computer work, interaction with patients, facility/hospital staff, and physicians.
Supervision
  • Assign oversight of LPNs/LVNs, RNs, Patient Care Technicians, and Home Therapy Care Team Assistants as a Team Leader or designated Nurse in charge.
  • Eligibility based on successful completion of all FKC education and training requirements for new employees, a minimum of 12 months experience as an RN, and 3 months experience as an RN in home peritoneal dialysis and/or hemodialysis.
Education and Licensure
  • Graduate of an accredited School of Nursing.
  • Current state licensure.
  • Current or successful completion of CPR/BLS certification.
  • Meet state practice requirements in the state of employment.
Experience and Required Skills
  • Entry level for RNs with less than 1 year of Nephrology Nursing experience in the last two years as a RN.
  • Minimum of 1 year experience as a Registered Nurse (preferred).
  • Home dialysis therapy experience (preferred).
