Performs technical and administrative work required to evaluate the necessity, appropriateness and efficiency of the use of medical services procedures within our acute care facility. Responsible for clinical review of all acute and outpatient services for appropriateness based on Inter Qual. This individual is responsible for the management of hospital resources necessary and appropriate for achievement of desired acute and observation outcomes for his/her assigned caseload, and the coordination of appropriate LOS along with assigned hospitalists.

• Assess appropriateness of inpatient admissions and stay utilizing Inter Qual; communicate findings to secondary reviewers, hospitalists, and MSW.
• Manage patient care across the acute and outpatient continuum, including admitting screens, discharge planning, and daily chart reviews.
• Provide clinical case management for medically complex cases, ensuring accurate admission orders and safe discharges.
• Participate in interdisciplinary team meetings to discuss changes in patient condition, admission status, level of care, and discharge plans.
• Utilize Inter Qual criteria for admission and discharge to validate medical necessity and reduce avoidable days.
• Collaborate with MSW/DC Planner for high‑risk and psychosocial cases, develop and implement discharge plans, and coordinate education for patients and families.
• Manage utilization and conduct Inter Qual reviews within 24 hours of admission.
• Collect data, track LOS, and report financial and utilization metrics to leadership.
• Act as a resource for all departments to ensure appropriate use of acute care services and appropriate unit assignment.

• Current California RN license; BSN preferred.
• Minimum 2 years of acute‑care nursing experience.
• 1‑3 years of Case Management or Utilization Review experience;
  Inter Qual familiarity.
• Previous case‑management experience in an acute‑care setting is preferred.
• Certification in Case Management preferred.
