As the Care Transitions Coordinator (CTC), you will be responsible for executing the sales strategy to increase company market share through account development and educating the medical community on services provided by the company while operating within a set budget. The CTC's primary responsibility is to facilitate a seamless transition for patients discharging from a facility setting to the care of our agency for post‑acute care needs.
You will work directly with the facility discharge planner to verify the receipt of orders and the agency's ability to meet the needs of the patient.
- Achievement of monthly personal production goals and MC admit budgets for assigned locations while protecting the company's financial resources.
- Execute a weekly, monthly, and quarterly strategy to increase market share within assigned facilities.
- Follow Right of Choice, evaluate patients and orders for suitability for home care.
- Initiate face‑to‑face patient transition to educate the patient on LHC agency and identify primary care physician to follow the plan of care.
- Present agency Executive Director with identification of patient needs to obtain branch approval and acceptance and complete CTC encounter documentation in Home Care Home Base.
- On acceptance, coordinate organization of transfer orders, other ancillary services (DME | Infusion) as needed, and educate patient on home care/hospice orders received from the referral source.
- Ensure all patient needs identified by the referral source are documented and met by the agency.
- Work closely with the Executive Director/Clinical Director to drive a vision of growth focused on the needs and expectations of the referral community and patients.
- Perform all sales administration duties, including BOA expense entry, payroll time sheets, PTO requests, and attendance at required sales calls.
- Educate patient on importance of post‑facility discharge follow‑up appointments, prescriptions, medication, pharmacy, and delivery method.
- Serve as liaison between LHC Group agency and all involved healthcare providers of newly referred and existing patients.
- Communicate to discharge planning any active patients that transfer from home health into a facility and coordinate resumption of care when applicable.
- Provide follow‑up feedback to case management team regarding readmissions and non‑admit decisions.
- Maintain patient confidentiality at all times.
- Know the features and benefits of LHC Group services, articulate competitive advantages, specialty programs, and Medicare guidelines; educate the medical community through effective sales calls and in‑services.
- Current and unrestricted RN, LPN, SW, or PT licensure in state of practice.
- Valid driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
- RT and/or technical school certification demonstrating solid clinical knowledge.
- 1+ years of home health or hospital case management experience.
- Thorough understanding of home health qualifying criteria and coverage guidelines.
- Excellent presentation, negotiation and relationship‑building skills.
- Solid computer skills to meet Microsoft Outlook and other software requirements.
- 1+ years of medical marketing experience.
- Excellent verbal and written communication skills with all members of the healthcare team.
- Excellent organizational skills and ability to complete competing priorities.
Compensation is based on several factors, including local labor markets, education, work experience, certifications, etc. Salary ranges from $48,700 to $87,000 annually for full‑time employment. Benefits include a comprehensive benefits package, incentive and recognition programs, equity stock purchase, and 401(k) contribution, all subject to eligibility requirements. The company complies with all minimum wage laws as applicable.
