We’re recruiting for a Care Coordinator to join an established healthcare provider in Brighouse, to assist in managing care resources across its Complex Care service. The role As Care Coordinator, you’ll play an important part in making sure care visits are properly planned, staffed and delivered. Working closely with the Service Manager and Complex Case Managers, you’ll coordinate care schedules, manage staff allocation and respond quickly when circumstances change.
You’ll also be responsible for monitoring training and supervision requirements, maintaining regular contact with clients and supporting the weekly on-call rota. This is a busy and varied role where strong organisation, communication and problem-solving skills are essential. What you'll be doing Coordinating care visits using the organisation's scheduling system Managing staff allocation and responding quickly to changes in staffing requirements Coordinating new client requests and making sure appropriate care is arranged Monitoring staff supervision, training and CQC-required timescales Maintaining regular contact with clients and helping ensure care plans are being delivered effectively Working closely with healthcare workers, Service Managers and Case Managers Recording and following up concerns, complaints and compliments Managing the weekly on-call rota and providing support when required Keeping accurate records and ensuring information is communicated clearly across the team What we're looking for We're looking for someone who is highly organised, proactive and comfortable working in a fast-paced environment where priorities can change quickly.
You'll ideally have:
Previous experience in care coordination, healthcare administration, scheduling or a similar role Strong organisational and time-management skills The ability to manage multiple priorities and respond calmly to changes Good understanding of CQC requirements and the importance of compliance Excellent written and verbal communication skills Experience working with care or healthcare staff NVQ Level 3 in Health & Social Care, or a willingness to work towards this A genuine commitment to delivering high standards of care Willingness to participate in the on-call rota What you'll get in return Basic salary of up to £28k Up to £3k additional on-call payments Quarterly team bonus 25 days holiday bank holidays & your birthday off Pension scheme Employee Assistance Programme, including wellbeing and financial support Buy-and-sell holiday scheme In-house training and development Supportive working environment with genuine opportunities for progression If you're an organised and people-focused coordinator looking for a role where your work genuinely makes a difference, we'd love to hear from you.
