What You Can Expect
  • Support both Home Health and Hospice
  • Complete psychosocial assessments and plans of care for home health patients.
  • Identify and provide interventions and goals as indicated by the plan of care.
  • Consult with physicians, healthcare professionals, the home health care team, and community partners.
  • Be part of the multi-disciplinary team.
  • Provide community resource information to home health patients/families.
  • Compensation:
    Paid hourly including drive time and mileage reimbursement for traveling or fleet car.
  • Sign‑on bonus eligible.
  • Professional development:
    Clinical supervision and in‑house educational opportunities.
  • Schedule:

    Monday‑Friday.
Qualifications
  • Education:

    Masters degree.
  • Licences/

    Certifications:

    Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in states of practice.
  • Other Requirements:
    Valid driver’s license and proof of auto insurance.
