SummaryThe Care Coordinator will provide outreach, navigation and coordination of care within the healthcare system for patients with Diabetes, Hypertension and those due for preventive services, with a focus on those who are uncontrolled. The Care Coordinator is expected to exercise a high degree of initiative and judgment in coordinating patient care and follow‑up.
Preferred:
Bilingual English/Spanish.
Pay: $16.50 – $18.50 DOE.
Quarterly bonus up to $520.
Minimum Qualifications- High school diploma or GED equivalent.
- Excellent communication skills.
- Good people skills to work with patients, clinical staff and specialists.
- Bilingual English/Spanish required.
- Medical terminology experience preferred.
- Proficient with Microsoft Office.
- Knowledge of local community resources preferred.
- Good organizational and time‑management skills.
- Ability to exercise discretion and maintain patient privacy.
- Ability to work independently and as part of a team.
- Valid Idaho driver’s license.
- Serve as an essential link between patients and all other caregivers.
- Facilitate patient referral appointments to specialty care and community‑based resources.
- Coordinate patient care and track support for patients.
- Follow up with patients who did not attend appointments.
- Follow up with providers to ensure results are received.
- Communicate effectively with providers, nurses, specialists, and patients.
- Act as a point of contact and reference for patients with questions or difficulties in their care and referral.
- Assist patients in resolving barriers to care (e.g., transportation, language) through collaboration with FHS and community resources.
- Ensure completion of patient records regarding referral outcomes.
- Follow FHS policies and procedures for documenting in the EHR.
- Assist with data collection, patient registries, documentation, and reporting.
- Collaborate with the clinical team, O & E, Care Managers, BHCs, etc.
- Participate in ongoing training, learning sessions, conference calls, webinars, and other professional development opportunities.
- Perform other duties as assigned.

Compliance (Medicare)Employees must follow FHS Medicare Compliance Plan and standards of conduct, participate in compliance training, and report any concerns or violations.
Patient‑Centered Medical Home (PCMH)Employees are expected to participate actively in the PCMH process.
Procedure ComplianceEmployees must read and understand all operational, safety, and environmental procedures related to their job.
Working ConditionsWork is performed in a typical interior/office environment. Limited physical effort and exposure to risk.
