Are you passionate about helping people achieve their goals and live their best lives? We're looking for a dedicated Service Coordinator with a Bachelor's degree in the Human Services field to join our team!
What You'll Do- Partner with individuals to create personalized plans and help them reach their life goals
- Meet face-to-face with your caseload at least once per month and stay connected via phone or in-person
- Link people to community events, activities, and essential resources like Medicaid, food banks, and free/discounted medical services
- Track progress, monitor satisfaction, and ensure individuals receive the support they need
- Complete timely data entry in electronic health records to document services provided
- Bachelor's degree in Human Services
- Strong communication skills and a passion for advocacy
- Ability to connect people with community resources and monitor their progress
