Een huisartsenpraktijk in Uithoorn zoekt een Praktijkondersteuner Huisarts (POH) om chronisch zieke patiënten te begeleiden en kwetsbare ouderen te ondersteunen in hun zelfstandigheid. Je bent verantwoordelijk voor het opstellen van zorgplannen en coaching in leefstijl. Je hebt een afgeronde opleiding tot POH-S en/of POH-O en bent sterk in communicatie en samenwerking. De praktijk biedt een aantrekkelijk dienstverband van 18-24 uur per week met mogelijkheden voor ontwikkeling en een salaris overeenkomstig de cao Huisartsenzorg.
