This is an exciting opportunity for an Advanced Clinical Practitioner to join an innovative and supportive Primary Care Network.

The post holder will work regular weeklysessions in the practices and be an integral member of the General Practice Team. This is different to many isolated PCN roles, it is an opportunity tobecome part of a progressive organisation where clinical excellence, teamwork,and staff wellbeing are central to everything we do.

The post holder will work within our 7member practices (Bassingham Surgery, Branston & Heighington Family Practice, Brant Road Surgery, Church Walk Surgery, Cliff Villages Medical Practice, The Heath Surgery, Washingborough Family Practice) along withundertaking work for the PCN. The post holder will mainly work across 3-4practices, with a need to cover other practices on occasion.

Main duties of the job

The role will mainlyconsist of providing a mixture of on the day and routine consultations whichmay be face to face, telephone, online or home visits. In addition, thepostholder will spend a smaller proportion of their time supporting the PCN byworking with our care home team. This would involve supporting with triage,telephone consultations and visits to care homes.

The role will require the ability to work flexibly to meet the needs of the organisation andthis may include working outside of core General Practice hours or within our Enhanced Access service.

As an Advanced Clinical Practitionerwithin our PCN, you will play a vital role in delivering high-quality,patient-centred care across our member practices.

You will:

  • Deliver autonomous clinicalassessment, diagnosis, treatment, and management of patients presenting withacute and chronic conditions
  • Prescribe independently where appropriate
  • Provide evidence-based care fora diverse patient population
  • Work collaboratively withinmultidisciplinary teams
  • Support service development and quality improvement initiatives
  • Contribute to the ongoingtransformation of primary care services across the network
  • Help shape innovative ways ofworking to improve patient outcomes and staff experience

Job responsibilities
  • Integrate and work within our member practices to enhance and support day to day access and capacity.
  • Provideadditional capacity support for the PCN care home team.
  • Assess and triage patients of all ages (including children), including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways or make necessary referrals to other members of the Primary Care team.
