We are seeking a dedicated and patient-focused General Practitioner to join our friendly and supportive primary care team. As a GP, you will provide comprehensive medical care, including consultations, diagnosis, treatment, preventative healthcare, and chronic disease management. You'll work alongside experienced clinicians, nursing staff, and administrative teams committed to delivering exceptional patient outcomes.

• Undertake face-to-face, telephone, and digital consultations for patients presenting with a wide range of acute and chronic conditions.
• Provide comprehensive assessment, diagnosis, treatment, and management plans in line with current clinical guidelines and NHS Wales standards.
• Prescribe medications safely and appropriately, ensuring adherence to local prescribing policies and formularies.
• Manage and monitor patients with long-term conditions, including participation in chronic disease management programmes.
• Promote preventative healthcare, health screening, vaccination programmes, and patient education initiatives.
• Make timely referrals to secondary care, community services, and other healthcare professionals where appropriate.
• Maintain accurate, contemporaneous clinical records using the practice clinical system in accordance with professional and regulatory requirements.
• Work collaboratively with the wider multidisciplinary team, including nurses, pharmacists, physiotherapists, mental health practitioners, and administrative staff.
• Participate in clinical governance, quality improvement activities, significant event reviews, and audit processes.
• Contribute to the effective operation of the managed practice, supporting service development and the delivery of accessible primary care services across the local community.
• Provide supervision to trainees, medical students, or other healthcare professionals where appropriate.
