Respiratory Advanced Clinical Practitioners

An exciting opportunity has arisen for two Respiratory Advanced Clinical Practitioners (ACPs) to join our expanding Respiratory Team and play a key role in delivering high-quality care across acute and outpatient respiratory services. The successful candidates will work as part of the wider multidisciplinary respiratory team to improve standards of care for patients admitted to hospital with Respiratory problems. Working with our established multi-professional team, the new advanced practitioners will enable a 7 day respiratory service to be stablished with care delivered as early as possible in the patient's journey.

You will be working alongside the existing team of Advanced Practitioners, Physician Associates, the resident doctor team and consultant colleagues. The role will help to support the consistent delivery of standards of care outlined in the National asthma and COPD audits, improving patient experience, care and flow. There will be a strong emphasis on evidence based care and the demonstration of this through audit completion to demonstrate that we are delivering care in line with the national standards.

Respiratory ACPs will undertake the assessment, investigation, diagnosis and management of patients presenting with a broad range of respiratory conditions, developing and implementing individualised plans of care and ensuring patients receive timely, evidence-based treatment.

• Provide comprehensive assessment, diagnosis and management of patients presenting with a wide range of respiratory conditions.
• Undertake timely review of patients presenting with respiratory problems following admission, improving adherence to evidence based standard of care
• Develop, implement and evaluate individualised plans of care in collaboration with patients, families and the wider multidisciplinary team.
• Work autonomously within their scope of practice while recognising when specialist or consultant input is required.
• Contribute to the delivery and development of a 7-day respiratory service, working collaboratively with the wider team of Advanced Practitioners, Physician Associates and other members of the respiratory workforce.
• Support the ongoing development and delivery of the respiratory virtual ward, helping to provide continuity of care and early identification of patients suitable for out of hospital care.
• Work collaboratively across inpatient and outpatient services to support effective patient pathways and reduce avoidable hospital admissions and length of stay.
• Contribute to teaching, service development, clinical governance, audit, research and quality improvement within respiratory medicine, Works as an autonomous practitioner taking responsibility for assessment, diagnosis and treatment of patients presenting within their care. Evaluate response to treatments and initiate changes as required within scope of competence. Work within own competence and confidence to ensure safe practice for patients and where unsure seeking out senior colleagues or medical team for support.
  
  Communicate effectively with patients, supporting people in making decisions, planning care or seeking to make positive changes. Communicate highly complex information to the Multi-Disciplinary Team (MDT). Work collaboratively with other professionals and refer to appropriate clinicians to ensure the needs of patients and services are met. Request and perform diagnostic procedures and support staff with patients who are unable to obtain these procedures.
  
  Request diagnostic interventions, including pathology and radiological tests within own speciality skill set whilst working within own remitof competency. Admit and discharge patients within own competence and utilising pathways, policies, and guidelines. Review of the deteriorating patient and initiation of management plans and escalation to medical team or senior colleagues where appropriate. Establish and maintain effective communication with patients, their families and other relevant agencies and professionals across health, education and social services.
  
  Promote clinical excellence and evidence-based approach to care by complying with the agreed policies and procedures. Provide support and advice to other specialities to ensure the correct patient pathway. Take an active role in Health promotion of individuals through appropriate support and education.
