Occupational Therapist - Adult Social Care

Working Pattern:
Community-based with flexible working options

Local Authority located in Herefordshire is seeking a qualified, HCPC-registered Occupational Therapist to join its Independent Living Service. This role focuses on supporting adults of all ages to maintain independence, wellbeing, and quality of life through strengths-based, person‑centred practice within the community.

• Complete statutory Occupational Therapy functional, social, and environmental assessments
• Apply a strengths-based approach to identify risks, barriers, and preventative strategies
• Support adults in their own homes to engage in meaningful daily activities
• Recommend and facilitate provision of equipment, minor adaptations, and specialist community resources
• Undertake moving and handling assessments promoting dignity, safety, and independence
• Work collaboratively with social workers, health professionals, housing teams, families, and carers
• Maintain accurate documentation and work in line with safeguarding and local authority procedures

• Degree or Diploma in Occupational Therapy
• HCPC Registration
• Experience working in Adult Social Care or community settings
• Knowledge of relevant legislation and person‑centred practice
• Strong communication, organisational, and caseload management skills
• Eligibility to work in the UK
• Enhanced DBS (assistance available)
