Adult Social Worker - Hospital & Discharge Team
Derbyshire County Council
Up to £23.66 per hour
Derbyshire County Council is seeking an experienced Adult Social Worker to join their hospital-based team in Chesterfield. This role is ideal for a practitioner with hospital experience who understands the Discharge to Assess (D2A) process and can manage complex adult cases effectively.
Key Responsibilities
Fast-Paced Hospital Assessment:
Conducting assessments to support safe and timely hospital discharges.
Complex Casework:
Managing a caseload of adults with complex needs across health and social care settings.
Discharge Planning:
Coordinating the Discharge to Assess process in collaboration with hospital and community teams.
MCA & Safeguarding:
Completing Mental Capacity Act assessments, leading safeguarding enquiries, and implementing risk management plans.
Care Planning:
Developing person-centred support plans and arranging appropriate services for individuals returning to the community.
Working Pattern
Office Presence:
Full-time on-site Monday to Friday, 9:00am - 5:00pm.
Requirements
Registered Social Worker (Social Work England)
Substantial hospital-based social work experience
Strong understanding of the Discharge to Assess model
Experience with complex case management, MCA, and safeguarding work
Excellent communication and multi-agency working skills
