Social Worker – Integrated Triage and Assessment Services

Newly Qualified Social Workers will commence on Grade 9 pay and job description before progressing to grade 10 after 2 years in post. Experienced social workers will commence at Grade 10 pay and job description. Both job descriptions can be viewed below.

This position qualifies for a market supplement payment of £200 per month. Please note this additional payment will be reviewed on 31 March 2026.

We’re looking for a dynamic and motivated Social Worker to join our Integrated Triage and Assessment Team. This is a unique opportunity to be part of a forward-thinking service that works collaboratively to deliver timely, person-centred support to adults and their carers.

As a key member of our team, you will:

  • Undertake strengths-based assessments with adults and carers.
  • Develop and implement care and support plans that promote independence, wellbeing, and recovery.
  • Work closely with health, housing, and community partners to ensure a holistic and integrated approach.
  • Focus on early intervention to prevent escalation of need and support people to live the lives they choose.

What We’re Looking For:

  • A qualified and registered Social Worker (Social Care Wales registration required).
  • Experience or interest in working with adults.
  • Strong communication and decision-making skills.
  • A commitment to promoting independence, dignity, and choice.
