Overview
The post holder will work autonomously in the community to manage a complex clinical caseload within the Service, applying specialist knowledge and skills relating to their professional role and working as an effective and efficient partner and coordinator in the interdisciplinary team. To work together with other professionals in health, education and social care to provide advice on effective care and health promotion to adults with stroke.
Within the Community Stroke Team the post holder will deliver by reason of their highly developed specialist knowledge, underpinned by theory, practice and experience, a high quality specialist service, providing expert advice, clinical assessment, triage, intervention, case coordination, and education to the support of patients and their carers. The post holder will provide support, guidance and supervision of less experienced professionals e.g. students, Assistant Practitioners (AP) and administrative worker.
They will work flexibly according to the needs of the Service.
- Undertake a holistic, comprehensive assessment of stroke patients (including screening and multi‑professional baseline assessments) with a complex presentation using investigative skills, analysing clinical and non‑clinical information, using appropriate formal assessment tools to determine their need for nursing and multi‑disciplinary service involvement.
- Act autonomously to assess, plan, implement and evaluate nursing interventions for a range of patients to address individual needs, including patients with highly complex presentations and those with multi‑factorial health and social needs.
- Provide specialist interventions based on a problem solving and advanced clinical reasoning approach; deliver interventions using a wide range of techniques; monitor, evaluate and modify treatment to measure progress and ensure effectiveness.
- Act as a clinical resource for the multidisciplinary team.
- Take responsibility for coordinating the interdisciplinary goal planning and discharge planning process; liaise with other agencies, initiate referrals, share information and recommend appropriate service delivery.
- Manage professional caseload efficiently, adjusting service provision to meet changing priorities and ensuring timely care.
- Coordinate day‑to‑day delivery of the service with the interdisciplinary team and undertake delegated managerial tasks as directed.
- Delegate appropriate caseload and tasks, supervise less experienced staff, manage performance issues, implement service supervision, e‑KSF and PDP, participate in induction and mentorship.
- Identify gaps and pressures within the service (waiting lists, staffing levels, resources) and discuss with the Team Lead to implement remedial action.

- Degree/Diploma or equivalent in Adult Registered Nursing.
- Registered with the relevant Health Professions Council (e.g., NMC).
- Minimum of 3‑5 years post‑graduate experience, including relevant experience working with adult stroke.
- Excellent interpersonal skills for dealing with patients, families and co‑workers.
- Ability to work independently and as part of a multi‑disciplinary team.
- Clean current UK driving licence.
- Experience working within a community‑based multi‑disciplinary team.
- Leadership and management experience.
- Knowledge of IT systems.
- Shift pattern:
Monday to Friday. - Salary is pro‑rata for part‑time hours.
