Role Overview

The successful candidate will undertake Continuing Healthcare (CHC) and Funded Nursing Care (FNC) assessments for adults with complex health needs. This will include face-to-face assessments across the Birmingham and Solihull areas, with report writing completed either from home or the office.

Key Responsibilities

  • Complete CHC and FNC assessments for adults with complex health needs
  • Undertake face-to-face assessments within Birmingham and Solihull
  • Complete DSTs, Checklists, Fast Track assessments and reviews
  • Work with patients, families, carers, advocates and multidisciplinary teams
  • Prepare clear, accurate and comprehensive assessment reports
  • Support CHC review panels and funding decision-making processes
  • Manage a complex clinical caseload independently
  • Liaise with care homes, local authorities, acute trusts and community services
  • Ensure assessments are completed in line with the National Framework for NHS Continuing Healthcare and Funded Nursing Care

Essential Requirements

  • Registered Nurse with current NMC registration
  • Strong experience in CHC / Continuing Healthcare assessments
  • Experience completing DST, Checklist, Fast Track and FNC assessments
  • Good understanding of the National Framework for NHS Continuing Healthcare and Funded Nursing Care
  • Experience working with adults with complex needs, long-term conditions, disabilities, palliative care or end-of-life care
  • Excellent report writing and communication skills
  • Ability to work independently and manage a complex caseload
  • Ability to travel across Birmingham and Solihull for face-to-face assessments

Desirable Requirements

  • Previous experience working within an ICB, CCG or NHS CHC team
  • Experience presenting cases to CHC panels
  • Knowledge of safeguarding, DoLS, Care Act and Mental Capacity Act
  • Strong stakeholder management and negotiation skills
