This Advanced Clinical Practitioner role will involve working alongside our multi-disciplinary team which consists of a GP Partners,Advanced Clinical Practitioner Partner and Pharmacist Partner. They are all supported by Advanced Clinical Practitioners who lead on acute same day care,alongside the Home Visiting Team, First Contact Physio, Pharmacists,Practice Nurses and HCAs. We also have Social Prescribers who work as PCNstaff dealing with the Mental Health of our patients.
The successful Advanced Clinical Practitioner willwork as an autonomous practitioner, diagnosing, managing, prescribing and discharging patients with illness or injury across all age groups including young children. This may be by face-to-face consultation, telephone or video consultation.

Main duties of the jobThe Advanced Clinical Practitioner (ACP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority toact and make autonomous decisions in the assessment, diagnosis and treatment of patients.
The ACPprovides an opportunity for patients to avoid the need to be referred to a GP,thereby enabling them to receive timely care and negate unnecessary delay in receiving treatment.
ACPs in primary care can develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results. Theycan independently prescribe appropriate medication, evaluate or refer to other specialists if necessary.

Job responsibilitiesPrimary Responsibilities:
The following are the core responsibilities of the Advanced Nurse Practitioner. There may be,on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
Workas part of a multi-disciplinary team to deliver safe and high-quality care.
Triagethe needs of patients, accordingly, making any necessary referrals for investigations in the appropriate manner.
Develop,implement and embed health promotion and wellbeing programmes.
Manage patients presenting with a range of acute and chronic medical conditions,providing subject matter expert advice both on site and during home visits.
Implement and evaluate individual specialised treatment plans for chronic disease patients.
Identify,manage and support patients at risk of developing long-term conditions,preventing adverse effects to the patients health.
Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
Request pathology services as necessary.
Processand interpret pathology and other test results as required.
Maintain accurate clinical records in conjunction with extant legislation.
EnsureSNOMED CT codes are used effectively.
Maintain chronic disease registers.
Chaperone patients where necessary.
Prioritise health issues and intervene appropriately.
Support the team in dealing with clinical emergencies.
Recognise,traige and refer patients presenting with mental health needs.
Bean extended and supplementary prescriber, adhering to extant guidance.
