Five Oaks Family Practice is seeking an enthusiastic and patient-focused Clinical Pharmacist to join our multidisciplinary primary care team. This is an exciting opportunity for a motivated pharmacist to play a key role in delivering high-quality, safe, and effective patient care within a supportive and forward-thinking GP practice.

As a Clinical Pharmacist, you will work closely with GPs, nurses, and other healthcare professionals to optimise medicines use, improve patient outcomes, and support the delivery of NHS primary care services. You will have the opportunity to develop specialist clinical skills, undertake structured medication reviews, support patients with long-term conditions, and contribute to quality improvement initiatives across the practice.

Main duties of the job

Key areas of responsibility will include:

  • Conducting structured medication reviews and medicines optimisation activities.
  • Supporting patients with long-term conditions through evidence-based prescribing and medication management.
  • Managing medication queries from patients and healthcare professionals.
  • Undertaking clinical medication reviews for patients in care homes and those with complex polypharmacy.
  • Supporting prescribing safety audits, quality improvement projects, and QOF-related activities.
  • Assisting with medicines reconciliation following hospital discharge.
  • Promoting safe prescribing practices and contributing to clinical governance activities.
  • Supporting the implementation of national and local NHS prescribing guidance.
  • Working collaboratively with community pharmacies, secondary care providers, and other healthcare professionals.

Job responsibilities

Clinical Practice

  • Undertake structured medication reviews (SMRs) for patients with complex medication needs, polypharmacy, frailty, learning disabilities, and long-term conditions.
  • Provide clinical medication reviews to improve patient outcomes and reduce medicines-related harm.
  • Support the management of patients with chronic diseases through evidence-based prescribing and medicines optimisation.
  • Assess, diagnose, and manage patients within their scope of competence and qualifications.
  • Develop and implement personalised care and support plans.
  • Identify and address medication adherence issues.
  • Provide advice and support regarding medication use, side effects, and treatment options.
  • Monitor high-risk medicines in accordance with local and national guidance.
  • Support care home residents through regular medication reviews and medicines optimisation activities.
  • Promote safe, effective, and cost-efficient prescribing across the practice.
  • Lead on medicines reconciliation following hospital discharge and outpatient appointments.
  • Review repeat prescribing processes to improve safety and efficiency.
  • Respond to prescribing queries from clinicians, patients, carers, and external healthcare providers.
  • Support the implementation of local and national prescribing guidance.
  • Identify opportunities to improve prescribing quality and reduce waste.
  • Assist with formulary compliance and prescribing incentive schemes.
