** We are looking for an experienced and motivated
** Physiotherapist Band 6
** to join our Integrated Neighbourhood Team based at The Orbital, North Swindon District Centre.
You will be delivering high-quality rehabilitation and therapy services within patients' own homes, residential homes, and nursing homes. Our community-based service focuses on helping people regain and maintain their independence, prevent unnecessary hospital admissions, and support timely discharge from hospital through person-centred assessment, treatment, and rehabilitation.
Working along as part of a multidisciplinary team of therapists, nurses, community support workers, and administrative colleagues, you will manage your own caseload and work closely with Therapy Support Workers to deliver effective rehabilitation programmes. **** Main Responsibilities
** As a
** Physiotherapist Band 6 you
** will provide a high-quality, evidence-based physiotherapy service to patients in the community, carrying out assessments, treatment planning, rehabilitation, and discharge planning to help individuals maximise their independence and achieve the best possible outcomes. Working as part of a multidisciplinary team, you will deliver patient-centred care, provide specialist advice and guidance, and support integrated working across health and social care services.
You will oversee rehabilitation programmes delivered by Therapy Support Workers, ensuring safe and effective delegation, supervision, and use of rehabilitation equipment. The role also includes contributing to the day-to-day running of the service through referral triage, workload coordination, and supporting the development of colleagues, while maintaining the highest professional, clinical, and communication standards.

* Diploma/ Degree in Physiotherapy
* Membership of Chartered Society of Physiotherapy and/ or evidence of other professional liability insurance to practice
