Job Summary

We have a fantastic opportunity for an enthusiastic, dynamic and experienced Advanced Clinical Pharmacist to join our Innovative Cardio Renal Metabolic  Medicines Optimisation & Cardiovascular Disease Prevention Group (Inno Meds) here at Leeds Teaching Hospitals NHS Trust. The Inno Meds group brings together multiple specialist teams to provide award winning cardiovascular care and research. This new and exciting post supports various applied clinical research areas covering the Cardiovascular Kindey Metabolic (CVKM) space.

We secured funding to pilot CVKM case finding projects and evaluate the impact of an integrated primary and secondary care MDT approach to better patient outcomes. This builds on the recently published NHS England CVD Modern Service Framework. The project is an applied research project with large clinical component. You will have the opportunity to develop your clinical and research CVKM skills among well established, award winning, multidisciplinary clinicians and researchers.

This post is 22.5 hours for 18 months fixed term contract.

Main duties of the job
  • Masters Qualification in Pharmacy (or equivalent)
  • Registrant of the General Pharmaceutical Council
  • Postgraduate diploma in clinical pharmacy or equivalent
  • Non-medical prescriber
  • Prior experience in working in primary or secondary care.
  • Good clinical pharmacy knowledge in cardiology
  • Ability to demonstrate foundation level competencies in all 6 domains of the Advanced to Consultant Level Framework (ACLF)
  • Substantial post registration experience as a practising clinical pharmacist.
  • Prior experience in delivering clinics
  • Expert knowledge in cardiology and or CVKM conditions (desirable)
  • Proven experience in medical education or clinical training roles (desirable)
  • Working across different specialties and healthcare systems (desirable)
  • Excellent communication and interpersonal skills
  • Strong organisational and project management abilities
  • Be highly adaptive to changing circumstances and situations
  • Familiarity with planning and conducting research
  • Development of educational materials

Why you should join our Team

The role involves supporting the steering committee of this CVKM project which has an additional name Cardiovascular Residual Risk Reduction (CVR3) project. You will be involved in the planning, developing searches in primary care, collecting data, reviewing patients, delivering clinics, optimising CVKM risk factors and medicines, attending MDT discussions, and contributing to the writing of the report of the project. There will be opportunities to develop your CVKM clinical knowledge, as well as your research skills.

You will be collecting data, publishing findings to evaluate and disseminate good practice. This position will ensure that patients are reviewed to the highest clinical standard with a person centred approach. In this role you will be expected to engage with healthcare professionals across West Yorkshire promoting an integrated model and linking with neighbourhood teams. The post holder will have the opportunity to support other projects within the Inno Meds group.
