Sirona care & health in England is seeking an Assistant Practitioner to join our community therapy team within Integrated Network Teams. The role supports patients to facilitate safe hospital discharges and promote independence in the community.
You will work alongside Physiotherapists, Occupational Therapists and MDT, providing holistic assessment and treatment with the wider team, and contributing to proactive patient care across planned and urgent referrals.
