HCRG Care Group in Swindon is recruiting an Advanced Clinical Practitioner (ACP) to join the Hospital at Home service. You will deliver acute, hospital‑level care at home, assess and manage patients, and lead clinical decisions within a multidisciplinary team.
The role requires NMC/HCPC registration, an accredited Advanced Clinical Practice qualification and non‑medical prescribing. A full driving licence and flexibility to travel are essential.
