Join our established Community Dietetic Team and play a vital role in delivering high-quality, patient-centred care across Wiltshire and Swindon. Every day, you'll help individuals achieve better health outcomes, maintain independence, and improve their quality of life through expert nutritional support in a variety of community settings.
As a Community Dietitian, you will provide high-quality, evidence-based dietetic assessment, treatment and management to patients across a range of community environments. Working as part of a supportive and experienced team, you will deliver personalised nutritional care to individuals with diverse clinical needs while contributing to innovative and responsive community services.
This is an exciting opportunity to develop your clinical skills across a varied caseload while making a meaningful impact on patients' lives outside of traditional hospital settings.

Contract: 37.5 hours per week (Full-Time)

- Managing a diverse clinical caseload, delivering high-quality dietetic assessment, diagnosis, treatment, and review for patients across community inpatient and outpatient settings.
- Developing and implementing evidence-based nutritional care plans, using clinical judgement to interpret biochemistry, anthropometric measurements, dietary intake, and medical information.
- Communicating complex nutritional information effectively to patients, carers, and healthcare professionals, using behaviour change, motivational interviewing, and counselling techniques to support positive outcomes.
- Providing personalised and practical dietary advice, ensuring interventions are tailored to individual needs, circumstances, and treatment goals.
- Working collaboratively with GPs, community teams, care homes, support workers, and other healthcare professionals to deliver integrated, patient-centred care.
- Assessing the need for oral nutritional supplements and provide specialist advice on nutrition-related prescribing and food-drug interactions.
- Supporting nutrition and hydration initiatives across community and inpatient settings, including menu review, nutritional standards, and partnership working with catering teams.
