Job overviewAn exciting opportunity has arisen for highly motivated individual to join our Community Nursing Team covering the North East locality of Shropshire in the provision of planned care to patients in the community.
We are looking for a caring, motivated and friendly Community Nurse to join our team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care.
Please note this position is community based, requiring the successful applicant to see patients in their own homes. Therefore, a valid driving licence with access to a vehicle for work purposes is essential.
Main duties of the jobYou will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development. We also actively encourage newly qualified nurses to join our team. In return we offer a preceptorship program and support professional development in a nurturing environment, allowing you to reach your full potential.
The team provides nursing care to patients within their own homes or chosen place of residence offering an alternative to hospital admission, supporting timely discharge and enabling people to realise their potential.
We are committed to clinical support, CPD & training. The post is covering 7 days, 8am to 6pm service.

- RGN NMC Registered
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Previous community experience
- Previous NHS experience
- Experience of working in a multi disciplinary / multi agency health/social care environment.
