All screens Nationwide Ltd Healthcare Assistant Frailty & Community Outreach in East & Central Brighton PCN provides support for frailty and outreach services across the area. The role includes planned reception duties at the Health Hub on Fridays and triaging patients on arrival.
You will work with a multidisciplinary team to coordinate care, support follow-up appointments, referrals, and signposting to local services, while maintaining confidentiality and patient-centered care.
