Leeds Community Healthcare NHS Trust is recruiting two full-time Band 6 Diabetes Specialist Nurse roles, with an option for Band 5-6 development for those with diabetes experience. The team includes nurses, dietitians, GPIs, diabetes facilitators and pharmacists across Leeds.
You will run diabetes clinics, conduct home visits, deliver education groups and support self-management for type 2 diabetes, with opportunity to complete NMP training if not already obtained.
