Location: Swindon
First City Nursing and Care in Swindon is seeking a Care Services Coordinator for a 12-month maternity cover. This full-time role (40 hours weekly) is based at the Swindon office and starts in September 2026.
Salary: £26,790.40 per year plus on-call payments, with a typical OTE of around £31,500. You will coordinate hospital discharges and community care, build strong professional relationships with discharge teams and health partners, and support safe, person-centred care when people return
