We are excited to recruit a Clinical Pharmacist to join our expanding multidisciplinary team as part of a new in-house pharmacy service at Hollyns Health & Wellbeing. This is an excellent opportunity to play a key role in developing and delivering an innovative, patient-centred pharmacy service that supports high-quality primary care.

Working closely with GPs, nurses, and other healthcare professionals, you will provide expert clinical pharmacy input to optimise medicines use, improve patient outcomes, and enhance prescribing quality and safety. You will undertake structured medication reviews, support the management of long‑term conditions, manage medication‑related queries, and contribute to the safe and effective use of medicines across the practice.

The postholder will contribute to quality improvement programmes, clinical audits, medicines optimisation initiatives, and service development, ensuring care is delivered in line with national guidance and local priorities. You will also support the implementation of evidence‑based prescribing and identify opportunities to improve patient safety and efficiency.

Main duties of the job
  • Contribute to the daily work of our in‑house pharmacy service, working alongside an Advanced Practice Pharmacist; supported daily by the medicines management lead.
  • To work closely with GPs, Clinicians and other practice staff and members of the primary healthcare team in developing your own skills to manage patient care and support high quality, safe, cost effective, rational, and evidence‑based prescribing.
  • Demonstrate safe, clinical decision‑making and expert care, including assessment and diagnostic skills, for patients within the general practice.
  • The post holder will demonstrate critical thinking in the clinical decision‑making process.
  • They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

Hollyns Health and Wellbeing is a proactive practice with two sites, who offer additional services to the practice population, such as warfarin, pain management, sexual health, Level 2 diabetic clinics and minor surgery.

Teamwork is integral in ensuring the service is run as effectively and efficiently for the patients as possible.

The successful candidate will be able to continue their service benefits if coming from the NHS in relation to the NHS Pension Scheme and annual leave entitlement.

The Surgery has
  • 12,300 patients approx.
  • GPs, Nurses, Health Care Assistants, Pharmacists, Advanced Clinical Practitioners, Advanced Nurse Practitioners, Womens Newborn & Infant Practitioner and Administration staff
  • Chronic disease clinics for diabetes, asthma and COPD
  • Rated Good by CQC
  • Active GP and Nursing training and teaching practice
  • Systm One clinical system

  • Part of a Primary Care Network with access to additional pharmacists, social prescribers, physiotherapy
Job responsibilities

Key Duties & Responsibilities

  • Medication reviews, computer based or telephone calls for more complex patients, first source of information for patients about medication.
  • Medication and treatment reconciliation systems for patients transferred out of and back into the practice, assess unplanned admissions and systems to prevent further unplanned admissions.
  • Practice formularies maintain.
  • QOF ensure data connectivity and proactive organisation to deliver maximum QOF for the practice. Regular QOF audits. Improve risk stratification of pathways and improve service delivery.
  • Medicines optimisation, support joint goal setting with patients and appropriate treatment selection to support better medicines experience. Reducing waste and improving outcomes. Follow up identified patient groups targeting better concordance leading to improved outcomes.
