The States of Guernsey are looking for a Senior Mental Health Practitioner to join the Older Adult Community Mental Health Team.

The Older Adult Community Mental Health Team, which incorporates the memory service based on the Island of Guernsey, is seeking an experienced Band 6 Senior Mental Health Practitioner. As a Band 6 Mental Health Practitioner, you will be part of a service providing specialist input to service users and their carers who may be affected by a mental health diagnosis. You will contribute to and deliver evidence-based, first‑class healthcare, prioritizing the needs of service users.

You are expected to possess knowledge and expertise gained through substantial experience with older adult community mental health needs, including assessing and supporting patients within the memory service and dementia care post‑diagnosis, underpinned by extensive post‑registration training to develop skills, knowledge, and values.

You will work autonomously in completing assessments, formulating person‑centered care plans, and conducting risk assessments for individuals accessing the service. Knowledge of Safeguarding processes, capacity assessments, and holding a valid driving licence are essential requirements.
